How to access Aged Care services in the Limestone Coast

Aged care refers to people over 65 years.

(Disability is managed under a different funding arrangement and as such this information does not transfer to those community members.)

If you are unable to manage your own needs independently and require support it's recommended that you:

Contact Access 2 Home Care on 1300 130 551.  This is the ‘gateway’ to booking an assessment to establish your aged care needs and what services you may qualify for.  Access 2 Home Care will take a comprehensive assessment over the phone prior to referring your request to the local Aged Care Assessment Team (ACAT).  (You must be approved for service before you can access aged care services, respite or Residential Care.)

There are many different types of aged care services available to support you, whatever your needs.

Once assessed and in receipt of your formalised approval for services you can then arrange to be placed on the waiting lists for the services you require.
